My patient may need Inpatient Rehab. How can I tell?

If your patient is suffering from any of the aforementioned conditions, the patient may be a good candidate for inpatient Rehabilitation at United Medical Rehabilitation Hospital.

Here's the rule of thumb when considering Inpatient Rehabilitation.
a. Does this patient have a condition that causes physical limitations and it is affecting their daily life?
b. Could their independence, functional status or mobility be potentially improved in a setting focused on physical rehabilitation?

If the answer to one of these questions is yes, your patient may be a good candidate. You can have your patient evaluated by our Nurse Liaison to see if they meet criteria. In most cases, the evaluation will be done within a few hours of the request. There is no charge for this evaluation and you'll have an answer the same day.
I'd like for a Nurse Liaison to evaluate my patient for Inpatient Rehabilitation at United Medical Rehabilitation Hospital.
What do I need to do ?
If your patient is in a Hospital or Nursing Facility, simply write orders to have the patient evaluated by United Medical Rehabilitation Hospital for Inpatient Rehab, i.e. a consult/eval order.
How will my patient's medical conditions be managed while in Inpatient Rehabilitation ?
If you choose, you may apply for privileges at United Medical Rehabilitation Hospital and make daily visits for the medical management of your patient. If you prefer not to follow your patient, a physician will be assigned to your patient while in the facility. Upon discharge, a follow-up appointment can be made with you for continuity of care.
